Dear Prospective Student:

Daley’s Municipal is please to announce we are currently accepting applications for EMT-Basic class. In this packet you will find the application for class along with the qualifications and requirements for acceptance into the class. Please read it carefully as the return of the material is time sensitive.

All required paperwork must be returned along with a non-refundable deposit of $500.00 no later than Friday,January 27^th, 2012. Applications can be dropped off Monday through Thursday from 9am until 4pm. Entrance testing, drug screen, and additional paperwork will be completed when the application is dropped off. Please plan on being here approximately 1 hour. Class size is limited to 20 and only those that submit the required paperwork and the deposit will have a seat held for them.

The student will also be required to obtain their own book package prior to the start of the class.
